Coordinating
our care with yours
Patients who come from long distances
are reassured to know that their
referring physician or other
healthcare provider will continue
care with them upon their completion
of treatment at Pacifica Pain
Management Services. Our entire
pain management program including
our one year of follow-up is
built upon this premise. We therefore,
make every effort to maintain
communication and cooperation
with the community practitioner
to whom the patient will return.
We take the following steps to
insure continuity of care:
- All formal written reports
are sent to physician
- A telephone call at the
beginning and completion
of treatment
- Optional updates at completion
of weekly case conference
- Discharge summary and
recommendations sent
to physician
- Progress notes during
year of aftercare follow-up
- Staff availability for
consultation during the
aftercare year of follow-up
In the event that the community
practitioner chooses to withdraw
from providing further care we
attempt to make arrangement for
referral within the patient's
area. |
